Planning for the future

Planning for the futurePlanning for the future is about talking with the people closest to you about what you may or may not want to happen regarding your care. Having a chat about your choices can be helpful especially if in the future you need to ask someone close to you to make decisions on your behalf. Remember this process is voluntary and it needs to be regularly reviewed as you may change your mind over time.

By having these conversations and writing them down you are making plans for your future care which can help the people closest to you to speak and make decisions on your behalf if you become ill. Your wishes and preferences are personal to you and people are there to support you to discuss these sensitive issues.

These conversations may cover a broad range of issues and many of these are covered in the Planning for the Future section.

Anticipatory Care Plans and Advance Care Planning are terms you may hear your doctor or nurse using.

Advance Care Planning usually refers to what you may or may not want to happen to you at the end of your life.

Anticipatory Care Planning is about planning for an expected change in your condition.
